When you start taking creatine, your weight may increase before you notice anything in the mirror or in your training results. However, the actual effect of creatine is not as dramatic as supplement advertisements often promise. It does not build muscle on its own or give you new strength with a single spoonful. The effect may be much more modest: one higher-quality repetition, a little more strength at the end of a set or better performance in the next set. Over the course of months, however, these small differences may add up to a noticeable result.
Most interestingly, creatine is no longer being studied only in relation to muscles. Researchers are increasingly interested in whether creatine's role in cellular energy metabolism could also affect memory, cognitive ability, coping with sleep deprivation and women's health. The preliminary results are exciting, but not yet conclusive enough to describe creatine as a universal brain or health supplement.
So what does creatine actually do, why can your weight increase while using it and for whom might its small advantage be important?
What is creatine?
Strictly speaking, creatine is not a protein, but our body synthesizes it from amino acids. Scientifically, it is a nitrogen-containing organic compound whose function is to support rapid energy metabolism in cells.
Where creatine is actually found in the body
About 95% of the human body's creatine stores are found in skeletal muscles, or tissues whose energy requirements can increase sharply. The remaining 5% is found mainly in the brain, heart and other tissues.
Foods also contain creatine, primarily red meat, fish and poultry. Plants contain very little creatine. Therefore, vegetarians often have lower baseline creatine levels in their muscles than omnivores.
Creatine's actual role
Creatine helps transport energy to where it is needed most at any given time.
Suppose your creatine stores allow you to perform 8 squat repetitions with a certain weight. During the final repetitions, phosphocreatine stores in the muscles decrease, ATP is no longer regenerated quickly enough and muscle power begins to decline.
If your muscles had larger creatine stores, you might be able to perform one additional repetition, maintain slightly more strength during the final repetitions or recover a little better between short sets.
This illustrates what creatine is used for as a supplement. The change during a single set may not be large. The benefits accumulate over time: if you can perform more high-quality repetitions over weeks and months, the muscles receive a stronger training stimulus.
Why can body weight increase after taking creatine?
Creatine can make muscles look larger and slightly increase body weight.
Why? There is a simple process behind it: when more creatine enters a muscle cell, the concentration of dissolved particles in the cell increases. To maintain equilibrium, water also moves into the cell to balance the difference in concentrations. This is called intracellular water retention. It is a process that slows down or ends as creatine stores decrease, meaning that this effect does not persist after supplementation is discontinued.
Therefore, your muscles may feel larger while you are taking creatine, and your body weight may also increase slightly. This is mainly water that your body is now retaining in greater amounts.
Creatine does not affect everyone in the same way. If a person's muscle creatine stores are already relatively high, the change may be modest, while someone with lower baseline stores may notice it more.
Does creatine help with weight loss?
The short answer is no: creatine is not a fat burner. It does not significantly accelerate metabolism or replace a calorie deficit. If you consume more energy than you expend, creatine will not make you lose weight. However, it may support training and help preserve muscle mass during weight loss.
Creatine may nevertheless support weight loss indirectly. A calorie deficit does not always reduce fat mass alone; muscle mass may also be lost. Creatine may help maintain strength during training and allow you to perform high-quality sets even when the body receives less energy than usual. Together with resistance training and sufficient protein intake, this creates better conditions for preserving muscle mass.
This distinction is important because the goal of weight loss should not merely be to achieve the lowest possible number on the scale. The recommended outcome is to reduce fat mass while preserving as much muscle mass as possible.
Because creatine may increase the body's water content, the number on the scale may not immediately reflect all the changes taking place. Therefore, when evaluating results, it is worth monitoring the long-term weight trend, waist circumference and training results in addition to body weight.

How much creatine should you take according to research?

Different forms are often marketed as offering better absorption, requiring a smaller amount or causing less water retention, but these claims are not always supported by direct comparative studies.
Creatine monohydrate is the most extensively researched form of creatine and is the form used in most studies examining creatine's effects and safety. Micronized creatine is monohydrate with smaller particles, which may mix better with liquids, but this does not make it more potent.
Creatine HCl dissolves better, but superior absorption or effectiveness has not been demonstrated. In studies, it has not produced better results than monohydrate in improving strength, muscle growth or body composition. Nor has buffered creatine increased muscle creatine content, improved training results or caused fewer side effects than monohydrate.
Creatine ethyl ester may increase muscle creatine content less than monohydrate. There are few independent studies on creatine nitrate, and its potential effect on blood flow may come from the nitrate rather than from better creatine absorption.
Based on current evidence, no alternative form of creatine has been shown to be better than creatine monohydrate.
Price comparison favors monohydrate
A spot check of prices in Estonian online stores shows that the price difference between forms of creatine is significant. The price per kilogram of unflavored creatine monohydrate was approximately 30–60 euros, creatine HCl cost about 100 euros per kilogram, and the price per kilogram of buffered creatine could exceed 300 euros.
The price is affected by the manufacturer, package size and promotional offers, but monohydrate was consistently the most affordable option across different brands. Creatine HCl and buffered creatine are often sold in smaller packages or as capsules, making them even more expensive by weight. Creatine ethyl ester and creatine nitrate are less widely available in Estonia, so their limited selection does not allow for a meaningful price comparison.
The practical conclusion is simple: based on price and proven effectiveness, the most sensible choice is unflavored creatine monohydrate in a larger package. Alternative forms cost more, but studies have not convincingly demonstrated that they are superior to monohydrate.
Creatine beyond muscles: new research
Creatine has long been studied primarily in the context of strength and athletic performance. Newer studies, however, are examining its potential effects on the brain, sleep, mood and women's health. Because the evidence in these areas is more limited than in sports research, preliminary findings cannot yet be turned into general recommendations.
Creatine's potential effects on memory and cognition
The brain continuously requires a great deal of energy and, like muscles, uses the creatine-phosphocreatine system to regenerate ATP. Researchers are therefore investigating whether increasing creatine stores could support memory, attention and processing speed.
A 2024 meta-analysis found that creatine supplementation had a small positive effect on memory, but the results were not consistent across all cognitive abilities. The potential benefit may be greater in situations where the brain's energy requirements are higher than usual, such as during sleep deprivation or aging. The effect was greater in women and in people aged 18-60 (Xu et al., 2024).
In one small 2024 study, a large single dose of creatine improved some measures of cognitive performance during sleep deprivation (Gordji-Nejad et al., 2024). However, the amount used was several times higher than the usual daily amount. Therefore, the study does not support the conclusion that creatine can replace sleep or that such a large amount should be taken without professional guidance.
Creatine and depression
Creatine is being studied as a potential adjunct to antidepressants and psychotherapy. A meta-analysis found a possible small to moderate effect on depressive symptoms, but the average change was not clinically meaningful and the quality of the evidence was very low (Eckert et al., 2025). Creatine must not be used as a replacement for treatment or as a reason to discontinue treatment; it is only a potential adjunct, not an alternative.
Creatine and women's health
Creatine's potential effects are being studied at different stages of the menstrual cycle and during peri- and postmenopause. At present, the strongest evidence concerns strength and muscle mass, while human trials on the menstrual cycle, pregnancy and perimenopause are still scarce (Smith-Ryan et al., 2025).
In a two-year study, creatine did not improve bone mineral density in postmenopausal women. However, potential benefits were observed for some measures of bone strength, lean mass and walking speed (Chilibeck et al., 2023).
Should you take creatine?
Creatine is not a magic button or the missing piece of a healthy lifestyle. If you eat a varied diet, exercise regularly and do not have a specific athletic goal, your body will most likely function perfectly well without a creatine supplement.
It is a different matter if you do explosive and intense training, want to improve your results considerably or want to preserve muscle mass during weight loss. In that case, creatine may provide a small but real advantage. Not one that you will notice in the mirror tomorrow, but one that may emerge through higher-quality training accumulated over months. The effect may be greater for vegetarians because the amount of creatine they obtain from food is usually lower.
However, one thing is worth remembering: creatine will not rescue a poor training program, compensate for insufficient sleep or build muscle without adequate nutrition. If the fundamentals are shaky, creatine is merely expensive powder in the cupboard.
Therefore, the best question is not “Does creatine work?”. Science shows that it does work for certain goals. A better question is: “Is that small advantage important for my goal at all?”. For some people it is, for others it is not, and both answers are entirely reasonable.
